BLAZE SSI creates and maintains software for TPAs and plan sponsors to
prepare and submit government forms required for employee benefit plans. The BLAZE SSI Government Forms Systems support preparation and submission of 5500 forms and associated attachments and schedules, PBGC
forms, 5300 forms and 1099 forms.
BLAZE SSI has received DOL/EFAST2 Certification for its EFAST2 Filing
Preparation and EFAST2 Filing Transmission software for the 2011 Forms.

Not all forms required for pension plan filings are transmitted through EFAST2. BLAZE SSI GOVFORMS Users who license the EFAST2 5500 Forms package will
also receive the IRS5500-EZ and 5558; the Summary Annual Report (SAR), the DB Annual Funding Notice and the PBGC forms preparation software. Under PPA06, beginning with 2008 plan years, plans insured by the PBGC are
required to prepare the Annual Funding Notice rather than the SAR.
PBGC Forms
PBGC Forms are electronically filed using 'MyPAA' and not with EFAST2. BLAZE SSI includes the PBGC Forms and schedules for current and prior years with its GOVFORMS 5500 Preparation software at no additional cost.
BLAZE SSI has received PBGC approval for the electronically filed 2012 PBGC
Premium Payment Forms preparation software and is listed by the PBGC as a vendor that meets the PBGC's current technical requirements. These forms are now available to licensed GOVFORMS 5500 users.
BLAZE SSI was the first vendor to receive PBGC approval and a vendor ID for
electronic filing of premium payment forms under the online 'My PAA' program. View the PBGC website for information about 'My PAA'. BLAZE SSI is
consistently among the first vendors to receive approval from the PBGC for new forms.
5300 Forms
5300 Series Forms are not filed using EFAST2. Software for these forms is available separately from BLAZE SSI at an additional cost. Currently these forms may be submitted in hard copy form to the IRS.
1099-R Forms
1099-R Forms are not filed with EFAST2. Software for these forms is available separately from BLAZE SSI at an additional cost. Currently these forms may be submitted in hard copy form to the IRS.
Licensing the BLAZE SSI GOVFORMS Systems
A GOVFORMS license is cost-effective for any size user regardless of the number of plans or system users. Single user, multiple user, multiple site and
