EFAST-Approved 5500s and other Government Forms Preparation Software
BLAZE SSI creates and maintains software for TPAs and plan sponsors to prepare and submit government forms required for employee benefit plans. The BLAZE SSI GOVFORMS System supports 5500 forms, PBGC forms, 5300 forms and 1099 forms.

BLAZE SSI 5500 GOVFORMS System users receive the 5500 Forms, the Summary Annual Report (SAR) and the PBGC forms all for one low price. BLAZE SSI users also receive prior years' 5500 and PBGC forms when they license the GOVFORMS 5500 System.

5300 Forms are available as an option to the 5500 forms. 1099 Forms are available separately.

2008 5500 Forms
BLAZE SSI has received approval from EFAST for the 2008 5500 bar-coded 'machine print'  forms. As of October 24, 2008 updates for the 2008 BLAZE SSI GOVFORMS 5500 System are available to BLAZE SSI licensees.

The EBSA filing system for the DOL/EBSA Form 5500, called the Employee Retirement Income Security Act (ERISA) Filing Acceptance System, or EFAST, requires that forms software vendors apply for annual approval of their software.

BLAZE SSI is consistently among the first vendors to receive approval from EFAST for 5500 forms. BLAZE SSI updates its users electronically to reduce the time it takes to obtain the new forms.

2008 PBGC Forms
BLAZE SSI has received PBGC approval for the 2008 PBGC Forms. These forms are now available to licensed GOVFORMS 5500 users who have renewed for the current year.

BLAZE SSI is the first vendor to receive PBGC approval and a vendor ID for electronic filing of premium payment forms under the online My PAA program. View the PBGC website for an introduction to My PAA.

Users who license the BLAZE SSI Form 5500 GOVFORMS System also receive PBGC forms and schedules for current and prior years at no additional cost.
